You receive your blood test results and see a sea of numbers, reference ranges in brackets, and perhaps a reassuring note that everything is 'normal'. But normal isn't always optimal, especially for women navigating perimenopause, menopause and beyond. The standard reference ranges are often built from population averages that include unwell people, and they rarely account for where you feel your best. Learning to interpret your own biomarker panel — and knowing which tests to request — is a powerful form of health literacy that transforms you from passive recipient to active participant in your care. A comprehensive women's vitality panel goes beyond the basics. Yes, a full blood count and thyroid function matter, but so do sex hormones across the cycle or at strategic timepoints post-menopause, markers of metabolic health like fasting insulin and HbA1c, vitamin D, iron studies including ferritin, and inflammatory markers such as high-sensitivity CRP. Many women are told their thyroid is 'fine' when TSH sits at 3.5 mIU/L, yet they feel chronically fatigued; functional and integrative practitioners often look for TSH closer to 1–2 mIU/L and will assess free T3 and free T4 alongside thyroid antibodies for a complete picture. Similarly, ferritin flagged as 'normal' at 20 µg/L may still leave you exhausted and losing hair — optimal for many women sits above 50–70 µg/L. Reference ranges tell you what's common; they don't always tell you what supports energy, mood stability, bone density and cognitive clarity. When it comes to sex hormones, context and timing are everything. A single snapshot of oestradiol or progesterone means little without knowing where you are in your menstrual cycle, whether you're in perimenopause with erratic cycling, or postmenopausal. FSH and LH rise as ovarian function declines, but they fluctuate wildly in perimenopause — one 'elevated' FSH does not confirm menopause if you're still cycling. For women on hormone replacement therapy, oestradiol levels are often best assessed mid-dose interval (not at peak or trough), and the choice of delivery method — oral, transdermal, vaginal — affects both serum levels and tissue effects. Progesterone, if taken orally as micronised progesterone, metabolises rapidly and may not show up reliably in blood; here, symptom tracking and endometrial protection matter more than the number. DHEA-S, testosterone, and SHBG (sex hormone-binding globulin) round out the picture, particularly for women experiencing low libido, muscle loss, or brain fog. Always discuss hormone results within the context of your symptoms, your stage of life, and your individual goals with a practitioner experienced in women's hormone health. Metabolic markers deserve equal attention, especially as insulin resistance often creeps in during perimenopause even without weight gain. Fasting glucose alone misses early dysfunction; fasting insulin, HbA1c, and ideally a glucose tolerance test with paired insulin measures reveal how your body manages blood sugar over time. Lipid panels are standard, but ask for particle size and number (ApoB, LDL-P) if cardiovascular risk is a concern — small, dense LDL particles are more atherogenic than large, fluffy ones, and this isn't captured by total LDL cholesterol alone. Inflammatory markers like hs-CRP and homocysteine help assess systemic inflammation and cardiovascular risk, while liver function tests and kidney markers ensure your detoxification pathways are supported. Vitamin D — crucial for bone health, immune function and mood — should ideally sit between 100–150 nmol/L for most women, well above the 'sufficient' threshold of 50 nmol/L used in population guidelines. Knowing your numbers is only half the equation; the other half is building a collaborative relationship with a practitioner who listens. Come prepared: track your menstrual cycle (or lack thereof), note symptoms with their timing and severity, and bring a list of current medications and supplements, as these can influence results. If your GP orders only a basic panel, you can request additional tests or seek a second opinion from an integrative GP, endocrinologist, or naturopath who specialises in women's health. Some tests — such as salivary cortisol curves or dried urine hormone metabolites — fall outside conventional pathology but may be valuable in specific contexts; discuss their relevance and limitations openly. Remember that herbs, nutrients and even dietary changes can interact with medications and affect biomarkers, so any supplementation should be guided by a qualified practitioner who reviews your full panel and health history. Optimal is personal. It's the place where your labs align with your lived experience — where energy is steady, sleep is restorative, mood is resilient, and your body feels like home. Reference ranges are a starting point, not a finish line.
